Registration Agreement

Informed Consent: I, the minor's parent and/or legal guardian, understand that Fit-N-Fun Bus, LLC is a sports activity and that by my child's participation in gym activities and/or moving around in the gym, with its equipment and possible uneven surfaces, there is a risk of injury. I understand the nature of these activities and the minor's experience and capabilities and believe the minor to be qualified, in good health, and in proper physical condition to participate in such activity. I give my child permission to participate in the programs offered by Fit-N-Fun Bus, LLC.

Liability Release Waiver: By submitting this online registration form, I, the minor's parent and/or legal guardian, acknowledge that I accept the risk and waive the option to sue should any minors for whom I am responsible for, incur an injury. By waiving the option to sue, I also thereby release Fit-N-Fun Bus, LLC and its agents or employees from liability for such injury.
